We all have doctors, hospitals and other facilities we prefer. Make sure they are in-network to maximize your benefit dollars. (Remember, the UHC plans each have the same network.)

What Will You Pay?
How much you’ll pay for care under each plan will be one of your deciding factors – especially if you’re thinking of choosing a new plan for the first time. Be sure to try to medical plan cost estimator before making your decision. This tool lets you input your personal information and get an estimate of how much each plan will cost you out-of-pocket.

Which Plan Offers You the Most Affordable Prescriptions?

As you consider whether to enroll in the PPO or HealthConnect Medical Plan along with the HSA, you’ll also want to compare your prescription drug costs under each plan. Express Scripts (formerly Medco) provides an online tool that can help you decide. Just go to www.express-scripts.com/gentiva and choose your plan name.

  • Review pharmacy benefit plan highlights
  • Find a local participating pharmacy
  • Compare prescription medication costs
  • Review the formulary (look up drugs by name)
