Welcome to the Gentiva team! You can access your 2013 and 2014 benefits information on the Harden benefits website until we all transition to the same benefits.

Is Your Doctor In-Network?
We all have doctors, hospitals and other facilities we prefer. Make sure they are in-network to maximize your benefit dollars. (Remember, the UHC plans each have the same network.)
What Will You Pay?
How much you’ll pay for care under each plan will be one of your deciding factors – especially if you’re thinking of choosing a new plan for the first time. Be sure to try to medical plan cost estimator before making your decision. This tool lets you input your personal information and get an estimate of how much each plan will cost you out-of-pocket.
