When you participate in the Gentiva Connections Wellness Program, you will not only benefit from a healthier lifestyle, you may also qualify for incentives to offset your medical plan costs. The incentive depends on the medical plan you enroll in for 2014:

  • If you enroll in the HealthConnect Medical Plan plus HSA (“HSA Plan”), you’ll get up to $500 (employee-only coverage) or $1,000 (spouse, child(ren) or family coverage) deposited in your Health Savings Account to save and spend on healthcare expenses — today, tomorrow, or whenever! The amount deposited is pro-rated depending on your Benefits Effective Date.
  • If you enroll in the UnitedHealthcare PPO Plan (“PPO Plan”), you’ll get a wellness premium credit of $10 every other week, up to $260/year. The amount you receive for the year depends on your Benefits Effective Date.

Take Your Health Assessment [50 points]

As a new employee, you can qualify for the Wellness Incentive for 2014 if you earn 50 points by completing your Health Assessment prior to your Benefits Effective Date. The Health Assessment is an online questionnaire that provides a personalized report detailing your health status, identifies areas of risk, and provides feedback on potential focus areas for improving your health. Individual responses and results are strictly confidential.

The Health Assessment is found under the “I Am” section of the Wellness website and should be taken after the Health Screening has been completed (so you already have your important health numbers).

The Health Assessment takes about 15 minutes to complete and must be done before your Benefits Enrollment Date so you can earn the 50 points required for the Incentive. If you’re ready to take it, log in to the Wellness site, click on “Health Assessment” under “I Am” at the top of the home page, and then click on the red “Take the Health Assessment” button near the top of the page.

Group results for the broad population will be shared with Gentiva to help our vendors design and implement health plans and wellness programs that better meet the needs of our employees, but your individual information can be seen only by you.

General information about whether you’ve completed the Health Screening or a physician visit will be shared with Gentiva only to confirm your eligibility to receive the incentive, but individual test results will not be made available to Gentiva without your consent.

Gentiva’s business practices are in full compliance with the privacy requirements of the Health Insurance Portability and Accountability Act (HIPAA).
